How does the service work?

Families can only access this service from Blakehill Healthcare if it has been identified as a need through a social work assessment. Once the child or young person meet the criteria for support, the allocated social worker will make a referral to Blakehill Healthcare with details of the details of care required. If we have capacity, we will arrange with the Social Services to contact the family and arrange a home visit.

At this visit, we will complete the necessary paperwork to ensure a comprehensive care plan is drawn up to meet the individual needs of the child or young person. The times and days to be provided will be discussed with family and agreed with a start date.

A Domiciliary Care worker will be identified based on the competencies required and an introductory visit will then be made with the family before the care starts.

Blakehill Healthcare provides trained staff to support children and young people from agees 0-18 in their own home. Support provided includes help with day to day activities including, washing, dressing and eating, support to get ready for school or college, support to relax at home. We also provide respite services should parents need a break.Blakehill Healthcare Children’s services are registered with the Care Quality Commission (CQC).
